U9 & Up All players must have a copy of player’s birth certificate
U9 & Up All players must have a picture of player
Payment – Check made payable to: GMSC
No U9 and up player registrations will be accepted without a birth certificate and picture
U9 & UP - ALL PLAYERS must have a PS1 form. Click HERE for form.

Cost:

$45 for M&M includes game t-shirt
$55 for U8 includes game t-shirt
$65 for U9 & up – new players must purchase a uniform from Sneaker Villa – Sinking Spring
$125 family maximum
$10 late fee for all registration received after April 23, 2009 (No late fees for U8’s or M&Ms)
No U9 and above registrations will be accepted after July 1, 2009
U8 and M&M registrations will be accepted up to the beginning of the U8/M&M season
